Support Coordination

exploring new options you might like or having a greater role in your budget – at a pace and level that works for you.
Our friendly Support Coordinator builds a strong relationship with you and provides information, resources and practical support to:
- Understand, implement and monitor your NDIS plan and goals
- Find the service providers that best meet your individual needs and present you with a number of providers who match your NDIS plan funded support requirements
- Connect you with appropriate services and activities that you choose, to live a good life
- Make sure you receive the right assistance during crises and vulnerable moments
- Engage effectively with your Care Team of allied health and key support professionals
- Increase your capacity for decision-making, independence and autonomy in the short and long term
- Ensure you are optimising your approved NDIS plan and are exploring a broad range of options
- Assist you to prepare for future NDIS plans (reassessment of your plan)
- Negotiate and establish service agreements with providers and to ensure services are meeting your needs by establishing the cost of supports and determining the supports they will deliver to you
- Ensure high quality Support Plans are in place across the services that work with you
- Direct you through the steps required for set up of myGov and the NDIS Participant Portal
- Build your confidence and skills to manage and coordinate services and budgets
